Yes, I see it quite frequently...well, maybe it's concentrated in certain fandoms? But since I'm reading for them rn, yeah. :|
And no, it's just for hospital. One says 'to hospital' and 'in hospital'. tbh it's not very hard to get a Britpicker or an Ameripicker or Canadapicker, so it just annoys me when people don't do it. I find it extremely jarring in fic (especially when people do ones that are super obvious, like 'pants' for 'trousers' ARGH).
